are they set to 2 or 1 weapon preference? though that shouldn't cause trouble, it might explain why the dwarfs are trying to keep their left hand free for a second weapon....?

The "pointy end, handle end" confuses many dwarves, they find the "strangle with bare hands" to be more on their level of intelligence  ;D

The wrestling comment before means if your soldier has wrestled an item away, he may be holding it in one hand.  That is making him hold the sword and shield in the other hand and causing him problems.  Drop his armor level down and dump any suspicious items he still has in his hands.  Then try to re-equip.
