Thinking size 40 wasn't enough for a predator, and making its bite do 3-6 gore damage. I embarked on a river to test them. Let's just say carp are officially extinct. However, condsidering the aim was to create something that actually derserved the "ability to make people afraid to go in the water" prefstring, I felt I actually did it all RIGHT.

Edit: Maybe the trapavoid and ambush predator tags were overkill...

From Mr. Welch's 1350 things he is not allowed to do in a RPG:
148. There is no Gnomish Deathgrip, and even if there was, it wouldn't involve tongs.
171. My character's dying words are not allowed to be "Hastur, Hastur, Hastur"
218. No matter my alignment, organizing halfling pit fights is a violation.
231. I am not allowed to do anything that would make a Sith Lord cry.
240. Any character with more than three skills specializing in chainsaw is vetoed.

I unwittingly dug one of my earlier forts out under an underground pond.

I found it while digging stairs upwards from my meeting hall.

I found out that water falls a lot quicker then it flows.

Training up Champion wrestlers and then giving them masterwork Steel boarspears (Like regular spears except they don't get stuck) before their armor was done.  And forgetting to set them to "On Duty".

At least it means I need to make less tombs.
Wow, they're basically like short range crossbows with infinite ammo.

Relying on insanity to dispose of elves for me, rather than drowning them in magma (THE PIPE WAS RIGHT THERE.)

I'm still convinced they caused that spiral.

I got introduced to this game with stories of flooded forts and things so I don't have any interesting watery deaths beyond people falling into the moat.

Biggest mistake I can remember was thinking "the wooden pumps will be able to pump magma long enough."

Not proparly noting my levers on a succession game.
The lever was connected to a gear, the gear to a pump that started a dwarvenly complex machine that caused the whole entrance to become a hell of repeatedly retracting spikes.
I added to that lever the note: "Pull to activate repeating spikes". I forgot to add "Do not remove this lever".

Another one, on one of my greatest forts so far, I had a 5-tile gold block road from the edge of the map to the middle, where my trade depot was standing under a giant, spiraling tower.
While looking around, I suddenly noted all my wells were dry. When checking the water supply, I saw a small tunnel going all the way from the well to the magma moat... I have no idea why or when I put it there. Some frames later, a wounded champion who was friends with half of my fortress died from thirst.
