They get stunned, and splat face down into a puddle.  While most things can hold their breath for a month, your legendary peasant who trained everything but toughness will likely drown in a few seconds.

It's also important to put grates (bars) so the dorf doesn't get swept downstream. That would be a dealbreaker.

I find from personal experiance, by just putting in one well and making so booze every year or two works perfectly, no slow dwarves and they don't spend 3 days holding a barrel. Provided you don't make too much booze, but then you can just drop the dwarf and booze into magma in the case he drinks too long.
by accident of course... ::)

Currently stuff will get swept past bars, grates, fortifications, anything. I guess that'll change in future versions.
