I honestly can't recall ever losing a dwarf to a sparring incident. It might've happen, but I don't remember it.

I've had a few cases where a guard or marksdwarf suddenly "has suffocated" (throat wound) or, more frequently, winds up in bed with a yellow wound, a rest job, and a huge thirst.
Mostly this was due to my own negligence ("Oh, immigrants ... a milker? p, C, fortress guard. Next? Oh, another animal caretaker? p, A ... ) ... while I forgot that some minutes earlier, I'd changed my top marksdwarves from "Crossbow, leather" to "Chainmail, Hammer" in order to get them some melee survivability. Which was somewhat optimistic as I didn't have enough chain and plate for everyone at that point ...

--> The result? A half-dozen brand-new recruits wearing cloth sparring against very strong expert marksdwarves who were only dabbling Hammerdwarves. Two of them got broken limbs by the end of the season.

Yeah, shields really make a lot of difference, no matter what material they're made of.

It does of course, and material matters. If your dwarves are decent wrestlers and shield users, the chance of being injured in training is quite small. [If they have the proper gear also of course..]
