So, I accidentally shut the dwarven caravan inside my fortress for a while last time they came, but let them out once I saw the message about the liaison going stark raving mad. The caravan left, and so did the liaison. Fast forward one year, and they're back...and so is the liaison, still insane.

This is...interesting, to say the least.

Seriously though, that's odd.

Not really odd at all. Afflictions of this kind are permanent in DF. If your liaison loses a leg during worldgen, he'll still be missing that leg when he turns up at your fortress. If he goes mad at your fortress one year, well, he's mad, so he'll still be that way the next year. I'm pretty sure this is all on the wiki.

Of course, one could argue about whether the fragility of the liaisons is bad from a gameplay perspective, but then you're getting into discussion about features, not bugs.

Actually, the question is "Wouldn't a mad liason start killing his own people when he gets home? Or massacre the caravan they're with?"

Well, he's not violently mad.

But seriously, don't you think that crazy bureaucrats would get pensioned off or something? Why do they keep sending him back? He's back again, this year, still crazy. And he'll probably manage to conduct his meetings just fine.
