Does "Virtually Never Sick" Guarantee Disease Immunity?

Can you get infected randomly for no reason like in real life?

Does anybody know how to become an elephant man in adventure mode? I literally did all advanced world settings possible to increase chances.

First, be aware that it is only a high degree of resistance, not total immunity. And there are no "random" diseases that show up "for no reason."

So far in unmodded DF, the only trouble that disease resistance protects against are wounds that become infected. For any wound that breaks the skin, the probability of infection depends on the dwarf's hidden "cleanliness" counter (recent bathing with clean water, soap), the quality of any healthcare applied (clean water, soap, doctor skill), and their disease resistance factor.

After infection, disease resistance also plays a role in whether a dwarf (eventually) clears a disease from their system, or lives indefinitely without ever fully healing.
