Exile to the hillocks. It's for the best.

It was inevitable.

Preventing bad thoughts is more important than giving good thoughts. Read a near mad dwarf's thoughts and see what is causing distress and remove that. Common causes can be over-working, a bad bedroom, not eating their favorite food. If it's combat stress - well, hillocks it is, then!

Giving them a long holiday in the tavern can help if it's in the early stage (red arrow only just appeared). But only with some dwarves. Others are lost causes, far too weak-minded. And you have to be careful with the vacation treatment as some dwarves get upset at not being able to craft or practice a martial art, etc.

Practicing a craft can be done in guilds now though, which is handy. Combined with priest sermons a dwarf might just about pull through.

Firstly, decide whether that dorf is worth the effort: if not, send off to a hillock treatment home.
If you're set on trying to save the dorf, you need to read, reread, and understand what the thoughts page says, and treat as many of the issues you can with individualized treatment, such as martial training, time off, crafting, friendship building treatment (similar to nuptial encouragement treatment), food and drink treatment, trinkets, clothing, room, etc. Be careful of "excitement", as the morons have a tend to pine for the excitement of battle, only to get into utter shock when seeing the corpse they just created...

It can be noted that dorfs that have mooded are immune to permanent insanity, and so can have episodes of stumbling around obliviously (beware of water cisterns!) for years before you start the treatment.

According to Loci's research, unmet needs don't actually have a huge effect on stress.

The best way to de-stress might be to lock them in a temple-designated room with a mist generator (and food/drink,) where they can't get into fights with other dwarves. You could also make it part of a legendary dining room and personal bedroom, but there's a risk they'll break the furniture. I guess you could set the locked door as internal and have all the valuable stuff outside in the hallway.

Maybe you could even have them do performances if they can see other dwarves through fortifications. (I don't think dwarves with perform with nobody around, but I could be wrong.)

Reading his name would trigger it. Thinking of him would trigger it. No other circumstances would trigger it- it was strictly related to the concept of Bill Clinton entering the conscious mind.
