How does title inheritance work?

I just had a new migrant inherit a barony - fine, I can arrange for an accident.  However, I noticed that his mother, father, older brother, and older sister are all still alive. How did he inherit the title?

If the old baron wasn't related to him, I bet he was picked at random, really.

What was the exact announcement you got? I believe it varies depending on how exactly the title was gained.

I didn't record the exact phrasing, unfortunately.

Surely inheritance can't be completely random - what's the point of having the system if parents never pass their titles on to their children?
