Mysterious Lung Damage

I am having dwarves suffer sudden and mysterious lung damage, followed by suffocation.

This is taking place in my well-guarded tavern, generates no combat reports, and has no obvious cause.
I haven't had and FBs or Titans pass by, so that's unlikely.

What could be the cause?

Alcohol poisoning.
To avoid altogether if you think it's a major issue, remove your tavern keeper and any performers. Dorfs apparently won't drink themselves to death unless assisted.

I assigned only a single goblet to the tavern, and I think it helps.

It is dwarven natural selection.  The ones who die must have had an elven/goblin ancestor.
