I put up with a vampire mayor for years, because I didn't want, as player, to "take justice into my own hands."

His killing spree, assisted by a forgotten beast, set off a tantrum spiral that left two adults living, one of them the vampire.

Somewhere, after the dust had settled and they were starting to rebuild, a dwarven child, unarmed and unaided, threw a tantrum and beat the incarcerated monster to death with her fists.

That was one of the most glorious moments I've ever had happen in one of my forts. 8)

So you could always hope that something like this happens...

Don't waste a perfectly good vampire. Build him a nice suite/office/prison. Put him in his own military squad and assign him some high quality armor so he doesn't end up naked and going insane because of it once his clothes rot away. Then seal him away and put him to work as your eternal bookeeper/mayor. He should require zero maintenance after his initial set up.

No, the dorf pulling a lever stands beside it, in my experience.

I just checked this, and the dwarf definitely stands on the lever tile.

I tried the suggested approach first and also had the issue of the dwarf standing on top of the lever to pull it.

I did still end up using a weapon trap linked to a lever. I got the mayor to stand on the trap tile after some serious messing around with burrows assignment. I placed the lever elsewhere, and my entire fort took turns pulling it. So they are all complicit in the execution that took place.

I'm going to let my other vampire out after his sentence is up. This means I'll be allowing him to kill again, and will then see if the hammerer does his job correctly.

Unfortunately in 0.42 having an isolated mayor is bad, since it means you can't receive petitions from visitors. I got really behind on petitions during my mayor's incarceration and had a big backlog once he was released.

I recently had the same issue. The slimy bloodsucker was lucky, and received 50 hammer strikes from an inexperienced hammerer. Although horribly wounded, with almost every body part damaged, it survived. So it is sent to the hospital.
Once it is in, I lock the door. What this clever mayor didn't know, is that for this particular bed in this particular room, I have built a huge garbage shoot right onto the bed. What ensued next was a stoning with 80% of the whole fort involved. There were probably ~50 marble stones dropped onto it. My dwarfs thought they were just cleaning up the newly dug residential quarters. Despite being injured, Mayor McVamp dodged about 30% of them, but one finally hit its spine/neck (can't remember) and then it couldn't dodge the remaining.

The problem of an isolated vampire mayor is easily solved by overriding democracy and appoint a mayor of your own choosing. This will have to be repeated every summer as the bugger is likely to be elected. While you're at it, you can select a non mandating mayor.

A bit harsh, perhaps, but it did kill one of my starting 7's only child and another dwarf.

When convicted, the mayor's sentence will appear as 50 hammer strikes.  The hammerer will come and get him, lead him to a restraint, and then immediately walk away.  When I check the justice screen, it says there is no more pending punishment. The hammerer is a competent hammerdwarf and is equipped with a +silver war hammer+.  No entries appear in the combat log, and the vampire mayor doesn't have so much as a scratch on him.
Plot twist, the hammerer is a vampire symphatizer, toss him into the lava after the mayor is dealt with!
