I did not update to 42.04 yet, but I searched "noble" in DF Announcement for both release 42.03 and 42.04 and there was no mention of issues getting multiple nobles in the same fortress (fort mode). 

At first I became a barony, then a season later, a citizen inherited into a baroness. 

Two years later, I was a duchy already with a Duke mandating things other than demands for room/office/dine/tomb.
Then I got 4 citizens inheriting 4 separate duke/duchess title.

Now I have 6 nobles (dukes and duchess) with different mandates and their usual room/office/dine/tomb requirements.

And they still work like normal citizens: Furnace Operator, Mechanic, Hunter, Axedwarf/Wood Cutter, etc.

Has this always been possible?

It was a side-effect of the outside world being simulated starting in .40. (Before that, no one inherited titles from elsewhere, because nothing happend in the world outside your fort.)

Hmm, I think I only have one single and the rest are a couple.  I wonder if I setup to marry two duchy nobles to each other--do the two duchy merge? 

And let's say they have an heir, and both parents die.  Does the heir inherit both duchy?  What happens to the heir's title, or name of the two duchy?

Succession isn't well understood.  About as much as we know conclusively is that dwarves can inherit titles from other dwarven cities in the world.

Also, if there is no dwarven king, a random dwarf in your fortress can become king.

Not true. I made an embark just to test something.  I left df in the background and came back to an annoucement that some people married.
