Simplest method?

R=ddddddddddddFddddu

R river
= channel (final step - have the rest done first)
d tunnel
F fortification (acts as a BD-proof screen)
u stairs up into your fort. Dig out the hospital on the level above and channel the the floor to open a lovely pool

If you are on flatland, you'll have to dig the tunnel down another level, just throw a diagonal link in the tunnel near the end to lower water pressure. Something like:
R=vddddddddFd/ddu
where v is a stair down and / is a diagonal tunnel step

the diagonal, from the top, would look like
         D
DDFD   DDU
DDFD   DDU
         D

Lots of ways to do this.

No it doesn't. Fully flooded fortifications are completely traversable, and partially flooded fortifications can be passed via "flow".

That's NOT what I wanted to hear (or maybe it is?).

Interesting that nothing has pathed through that set-up in many, many besieged forts. Ah well, grates out of reach it it. Or the 20 Z-level waterfall into the lovely viewing pool.
