I need the code for 3 inputs. 1 for username, 1 for email and 1 for the actual bug report/feedback. I don't know how to then arrange that form so that it emails a selected email address

You can't really without doing server side stuff.  Javascript can't send email.

You could pop up an email thing though:

You risk being found by spambots though.   :P

Tried it. it opens up a blank screen. I have yet to learn any javascript in case it relies on that.

Hey,

for the reporting itself you could use google forms. Just place a link with "Report a bug" on your page linking to the form. The data will be collected inside of a google spreadsheet.

Here is an example I set up in a few minutes.

Don't use this, I will delete it afterwards.
