Top reasons dwarves won't touch something:
* Reversed inside-outside bug. Enable outside refuse hauling from the workshop orders menu.
* It's forbidden. Use the mass un-forbid selection tool over top of it to make sure it really isn't forbidden. Never use the mass forbid tool, you'll forbid stuff you didn't intend to.
* It's owned by somebody. Ownership will eventually go away, but until then, you're SOL. The owner will sometimes put it away if they have a room and you disable their labors.
* Nobody has the appropriate labor.
* Everyone with the appropriate labor is busy with other jobs, on break, or trapped and starving on the wrong side of a wall they built.
* There's no path to it or where it's supposed to go (forbidden doors in the way?).
* Either the job item or where the job item is supposed to go is not inside of a worker's burrow.
* There's no appropriate stockpile with the correct settings or free space for the item, or if it's marked for dumping, there's no active dump zone.

How to fix it regardless of the problem:
* Wash it away with water or cave-ins. To get rid of it entirely, push it under a drawbridge.
* Incinerate it with magma or fire.
* Channel under it until it's out of the way or you hit magma.
* Drop a natural wall on it.
