suffocation

one of my guards just suffocated, but he was asleep on his bed at the time. He sleeps in a barracks, and there were multiple other dwarves in the room sparring and shooting at targets, and they are fine. How did this happen?

Sometimes a dwarf get hurt in the throat sparring but takes sometime to suffocate.
