It's not a werecreature I encountered in fort mode, but one whose image on an item I saw in adv mode.

It's a were-monitor.

Once, I had a were-platypus. Another time, a were-giraffe.

The infamous were-ass did a 40.05 fort in quite thoroughly. Arrived to a thriving fort, bit my mayor and killed my military. Then turned human, triggered a weird loyalty cascade/tantrum thing with my 'gang of ten cheesemakers armed with lefty and righty', and terrorized them all.

The mayor turned and infected three more dwarves and killed *seventeen*. At that point I gave up.
