I was desperately equipping my militia during a necromancer invasion when I noticed I had the option of equipping some weapons and shields that I definitely didn't make (there were wooden spears), and these unexpected items had brackets around their names.

I didn't get a good look to see if my militia had equipped these weapons, and soon the fortress fell. following this, I have a considerably better armed militia in my new fort, but still have the weapons/shields in brackets.

I have two questions:

1) What do the brackets mean?

2) Where do the weapons come from? (I can only assume from the woodenness that they were made by cave dwelling sentients and were tracked by the game after being left lying around by their owners.)

Sounds like you have or had a tribe of animal men in one of the caverns.

If by brackets you mean parentheses (), they mean the items were not made by your dwarves.
If by brackets you mean braces {}, they mean the items have been forbidden, so your dwarves won't pick them up until you unforbid them.
