I had one start in my 70+ year fort.  It was due to friendships created by a small memorial hall.  Two of the most popular dwarves died at the same time due to old age.  One of their friends got upset and went berserk.  When soldiers had to take him down, the rest of the fortress erupted.  I went for ~140 dwarves to ~70 :(.  The legendary dining hall, the private engraved bedrooms, the masterwork food, the fine drink, nothing mattered.  That memorial hall had created just too many friendships.  Most of the deaths were civilians.  Most of the 40+ military "didn't really care about anything anymore."  The fortress had all content dwarves within a year of the spiral, but it's still recovering population wise in its 8th decade.  I no longer have any specific gathering places of any kind.

80 year fort? that is serious dedication!
My current fort is only 19 but I haven't had tantrum spirals yet, the most serious catastrophe was 27 deaths during a siege and then only the baroness went mad. The fact everything is engraved does wonders for their mental sanity.

To return to the original topic, try using the DFHack 'misery' command; it increases the damage bad thoughts provide, making everyone more unhappy, and more likely to go beserk.

Since I like for forts to end in some amusing manner, making friends is something I try to let my dorfs do once the fort can survive without parties dooming everyone when the brewers all go party hard while dehydrating. Last fortress of 210 dorfs ended when Spring rolled, and 5 dorfs died to old age. Within a month, 80 dwarves were miserable, and pretty much 95%(excluding the soldiers who never make friends because they train so hard) of the fort was one way or another.

The tantrum spiral lasted a year before it ended with the last miserable dorf finally calming down after talking with the then elected mayor, because the previous ones had all committed suicide. About 50 of the original fortress survived. 15 migrants showed up to help clean up the mess, dump the bodies in the catacombs, and I abandoned the whole place since even with that few dorfs, the FPS was still around 15.
