I don't know what would be causing it, but you could prevent it by moving everything inside, the making a bridge connected to a lever and pulling the lever to it blocks the entrance. Then, no more carp deaths(Unless its in an undergound lake or something...) and no more kobolds. Though placing a guard at the entrance(Just use a peasant) stops the kobolds more often then not.

If the message was something like 'Dwarf cancels Drink: devoured by hideous demon fish' then it's possible they just decided to take a scenic stroll past the water on the way to the booze stockpile and weren't actually drinking it.

I've had this happen before too, except it was undead horses eating drinkers, not carp.  I had like 200 or something alcohol, but they headed out to the river in the middle of the Terrifying plains.

That's why I've locked the doors and don't intend to open them again. Ever. I'll work out a complicated airlock system to let in the traders.

Ouch @ the undead horses. I actually had a well build right at the exit and that seemed to stop them from being interested in river water. So, problem solved.  :D

Now, that trader trapping idea... If you could just hold them long enough to be useful but let them free before the melancholy happened... That could be helpful.

Also this can be caused by the lack of barrels. I mean, you have a lot of alcohol and barrels but if # of barrels < # of dwarves there can be a situation when all barrels are busy (dwarves do not use mugs)  so other thirsty guys decide to go take a drink from that carp-infested river.
