I once walked into a city, and saw a report of combat. Zombie Tiger charges at Human Potter. They ended up killing it somehow, but I got credit just for being around... Maybe same happened to you?

I did literally nothing, just walked between soldiers on the town wall asking for quests and getting them.

I don't think I've ever attacked my first bandit camp with 16 companions before.

Are you entirely certain that your adventurer was not, in fact, a master ruseman?

I get the distinct image in my mind of a man strolling into a keep filled to the brim with soldiers and regaling them with stories of all of the horrible beasties he has personally put an end to. Once he has their full admiration, he uses this new-found trust to convince them to follow him into the lion's den.

Nah, it's all sewage.

No seriously, they tend to spawn directly in the sewage tunnels that are submerged with no platforms around, and very obviously drown in a few turns. Try using DFHack and see for yourself.

It gets better. Some of them spawn into walls or the soil around the sewers. Dunno how they survive, the one time I saw it they were Fallen (Genesis mod), so unlikely to drown.

Yeah, I saw a goblin vampire trapped in a sewer wall, couldn't get out. It was fairly amusing.

Then I got ambushed by a Giant Desert Scorpion, ran all the way to the nearby evil biome and jumped in the water, throwing water at it until it died. Then I left, went to sleep, and got ambushed in the night by exactly two goblins, and died. :(

I learned that by making every race a reptile man, and was able to swim to them with out drowning, I am surprised that was normal for them to spawn in water.

I just stumbled over this one again: started a new character, chatted with a villager and got a 'report success' option. The reaction was 'tales from faraway lands won't impress here'. I think this one was due to the previous adventurer not having reported all her successes, so the new character got to tell the tale.

Outlaws in towns are also frequently torn apart by the townspeople themselves the moment you get anywhere near them. The locals will then hail your heroism when you make them aware of who it was they just chewed up.

This thread title makes me want to say "just add water."
This has also happened to me many times before, but I've never been able to pinpoint the cause. My two main suspects used to be:
1.Some weird bug related to retired adventurers
2.Possibly caused by raised zombie weirdness, when the zombie companion kills something, or when you kill raised zombies.

This tends to happen in towns, because sometimes, ringleaders are spawned in the sewage, not on the platforms that usually surround the place. They even spawn within the walls.

Can't speak for mods, but normally, the game is careful to only place amphibic foes in the sewers - amphibian/reptile/serpent/cavefish people and vampires. Those will not take harm from being placed in the water, and vampires don't even mind being stuck in walls.

I'm also fairly certain that a creature must be within a sort of 'activity' radius around the character, or they'll be 'offloaded' - the game saves them to file and doesn't update their health until re-activation. A bandit in the sewers on the outskirts of town should be unable to die while your character is in the keep in the centre.

This strange sequence of several 'successes' becoming reportable while talking to soldiers sounds rather like there's some sort of civil war going on within the keep itself, or in the dungeon below. I have seen bandit factions going and beating up each other.

Well, there's also the problem with bandits. They usually don't like each other, and because more than one bandit group can live in the same dungeon, well...
