That report snippet almost looks like the wheelbarrow, full of stuff, was falling and the contents, and barrow, hit a dwarf.

While I don't know if it matters for this, mentioning in the beginning that you are using a mod is important for troubleshooting, as sometimes its a case of "oh, yeah, that happens if you use that version, but not in vanilla."  Failing to mention use of a mod is like looking for help with the steering in your car, then after 30 minutes of discussing the problem with a mechanic you mention one of your front tires was lose.
