I have a dorf who was killed right up against the edge of the map.  I think that's the problem.  At any rate, they can't seem to bury the guy, because I keep getting "Urist cancels place Item in Tomb: Item inaccessible"
I get this 50 times a second, it seems.

Ideas for how to fix it??

I had that annoying spam as well.  I just toggled the free coffins off/on for burial.  That seemed to eliminate the cancellation spam.

yeah, i just noticed that his body is up on the battlefield, but the coffin reports itself as his final resting place...

The coffin has been claimed by his Dorfy spirit. Whether or not the actualy corpse bits are in the coffin is another matter entirely.
