It appears that if you have created a tomb with many coffins, sometimes you obtain spammed messages of "unable to put item in tomb."  If you remove the tombs, there are certain tombs that appear to have limbs or bones underneath them, and the messages stop.

Were you blocking off some tombs with other tombs?  Dwarves can't/won't walk over them.

The 'revealed bones' thing is just because they're emptying them out when they take them down, I think.

I have a lot of these messages (very annoying, and the lost jobs as well), but I think it can be attributed to a drowned Dwarf in a river channel that cannot be dried. Why don't corpses rot after a few years?
