So I knew something was happening because my frame rate frose and DF nearly crashes but I manage to boot it back up again.  I notice that the game has paused itself but no invaders or migrants have arrived.  So upon looking around I find that my barracks that I was trying to turn into a waterfall room with floor grates all over the place has collasped (partially) leaving the remaining squad members inside alone and very unhappy.  I  noticed that before this happened some of the water below had turned to ice. . . but other than that I don't know how this happened.

If your waterfall is fed from outside, then the tile where the water falls, the grate, and the tile beneath that are all water. Your framerate froze because all of the surface water on the map was freezing, which takes a minute. The cave in was caused by a unit of water suspended in the air turning to ice and crushing your floor grate, which likely caused more Fun.

That's basically all I can think. If more than just a few tiles disappeared, though, I'm not sure.

Well the waterfall wasn't in action yet. .. .

Floor grates DO NOT provide support for construction, so one floor grate will not hold up another. But the game will still let you try and as soon as the 2nd grate, or what ever you are trying to build connected to only it, is finished building it will collapse.
