I know on the fast travel screen lairs are like black ovals but when I get to the area where it says I can't fast travel (so I think I'm in the lair) I can't find the monster that I'm suppose to kill for a quest. So I don't know if I'm even in the lair yet.

The wikia says that lairs have doors but I see absolutely no doors anywhere. :S

Any tips?

there is a list on the top left of your screen. it tells you the location of certain things INCLUDING lairs/quests. the lair will be marked as a grey oval. it will tell you to go north,east,south,west,northwest,northeast,southwest or southeast to find the lair. sometimes lairs are underground, and the entrance is a hatch. to go down the hatch, stand on it and press >
 
P.S this little tool can be useful when looking for towns while you cannot travel (under attack by bogeymen)
