Just as it said. My hunter used up all his ammo, no one ever recollected it and put it in the pile. Now he died because he has no ammo left.

Also, i have it set to collect refuse from outside.

I also have a ammo stockpile built.

After an arrow gets shot it gets forbidden. Thats not a bug. A few versions ago dwarves would run out in seiges to pick up a single bone bolt and turn into a pin cushion for all the goblin arrows.

That's because stray ammo shot from a bow or crossbow is automatically designated as forbidden. This was to prevent silly dwarves trying to retrieve said ammo while under siege from goblins or other non-dwarven races. Not to mention giving hunters and marksdwarves a greater chance to take a bigger stack of bolts along when they do their job; otherwise they would use up the one bolt, then move in to melee range and bap them with their crossbow.

If theres a seige shouldn't you set the game to only allow soliders outside?

So its not the games fault people were collecting arrows outside, it was the player.
