I've had this happen. I was building an above ground fortress, creating a roof connected to the ground by a 3 Z level, single tile, stairwell. Suddenly one of my masons building the roof was standing on some walls that were not quite adjacent to the stairwell. I thought somehow he'd fallen from above, but he wasn't injured.
