ive found a dragon in layer 1 before, but then it "dragon name has vanished". I checked world gen and it was gone.

Ive got down to layer 3, but never hell. How do you get to hell with out tunneling in fortress mode?

I found demons killing refugees on the surface once (the fort they were fleeing from never breached hell).

I suspect you'll find those kinds of entrances (or at least 'access', it may not be particularly walkable terrain) in areas, and worldgens having such areas, where embarks have encountered.... 'strangenesses'.  As long as there's not a slightly different way of handling the loading of embark tiles, which means that it doesn't go wrong (or in the same way).

(I'm wondering, as I suspect this would be related, if anyone has wandered by any of those 'spires' in Adventure mode?  I tend to only hear of them in the Fortress forums.)

Yeah the one time I got into the caverns (found a cave on the beach purely by accident while I was en route to my umpteenth bandit camp) the most combat action I got was clearing out the troglodytes who lived in the passageway connecting the cavern system to the surface cave. They either need to drastically ramp up the amount of wildlife down there (which might make things a little too fun for new players in Fortress Mode), come up with some random ambush generators, or start having humanoids give underground quests.

So false. Try wandering around down there for two dwarftime days. When I was on my little quest to find a cave with a reachable exit after being knocked down there, I killed about 150-200 enemies, mostly trogs and crundles, with lots of pond grabbers and a good mix of harder foes.
i spent a day or two wandering around the third cavern layer looking for hell and never saw anything, so i guess it depends.
