Arcanum is a brilliant game with loads of stuff to do. It's got a similar playstyle to Fallout with an option to play Fallout Tactics style (don't do this, it's awful!), though I was less awed with it than Fallout I consider it to be one of the best RPGs for the PC. :)
If you decide to get it be sure to get the unofficial patch from Troika's old site, the game is pretty broken without it.

Might I recommend one currently in beta? Caves of Qud. Quickest way to describe it: Post apocalyptic dwarf fortress with semi static map focused entirely on adventure mode. There is fast travel across a world map, or you can play the entire game without visiting it once. VERY easy to get lost if you don't go to the fast travel map though.

If you're ok with a MMO, some of the Ultima Online freeshards are really good, I mostly play on the Excelsior shard myself, no skill cap and accelerated skill gain.

Actually, I have two worlds, but never actually tried it. The dialogue made me quit within twenty minutes. I couldn't stand it; it seemed like a off-brand Oblivion. Is it worth getting in to? I'm willing to look past the utterly horrible dialogue if there is some amazing game play behind it.

Have you played the first fable game, I really enjoyed that when I first got it, and I think it is better than the other two

Would X3-Terran Conflict count? Or am considering it a roleplaying game due to the random roleplaying I do during long flights. >.>

How about The Witcher? Pretty big, plenty of exploration. It's a bit more story driven than most of the games you've mentioned. There are "chapters" between witch you can't really move freely but there are many sidequests within them.

I bet someone already said Arcanum but I'll mention it anyway.

Try Nehrim: At Fate's Edge, a total conversion mod for Oblivion.  It has German voice acting but is translated into English.  Supposedly it's everything Oblivion wasn't (like no level scaling).  I'm sure this is exactly what you are looking for - it's supposedly one huge world loaded with hidden treasures for the diligent explorer.  Of course, there's a main questline, but you can complete it at your leisure.

Also, it's free if you already have Oblivion, so that's a huge perk.  It's supposed to be exploration pornography, basically.  Take a look at their website: http://www.nehrim.de/galerieEV.html
