Loyalty cascade!

A creature who kills a member of a civilization becomes an enemy of that civilization. Members of that civilization will attack enemies of that civilization. Trouble arises when the creature who killed a civilization member is a member of that civilization; that creature will be attacked by other civ members, who then become civ enemies, so they, too, get attacked by other civ members, who then become civ enemies... Repeat until everyone is enemies of everyone, or you somehow manage to stop the cascade.

Long story short, don't kill members of your civilization with kill orders. Arrange an Unfortunate Accident instead.

It's not about the murderer becoming crazy, it's about him attacking a member of your civilization and being flagged as an enemy. Unless you keep him away from the rest of your fortress somehow, they will attack him and, very possibly, be flagged themselves as enemies of your civilization, so on into infinity.

If he had gone crazy, he would no longer have been flagged as a member of your civilization (as far as I know anyway), and therefore would have been killed without any ill effects.

Here's how it works:

Your soldier is a member of your civilization and a member of your local group.
Your outpost liaison is a member of your civilization, but not a member of your local group.

When you ordered the soldier to attack the outpost liaison, the soldier became an enemy of your civilization but remained a member of your local group.

When other members of your fortress saw the soldier, they treated him as a renegade (and rightly so) and attacked him. However, upon attacking him, they became enemies of your local group but remained members of your civilization.

Effectively, this sort of loyalty cascade results in 3 factions - lawful citizens, enemies of the nation, and enemies of the state. Lawful citizens who attack enemies of the state become enemies of the nation, and lawful citizens who attack enemies of the nation become enemies of the state.

In closing, What The Hell, Player and/or You Bastard. Consider yourself lucky that Toady didn't explicitly check for this situation, otherwise you'd probably end up with a permanent war with the Dwarves and possibly the Humans and Elves as well (and perhaps to add insult to injury, peace with the Goblins).

P.S. If you don't get this note, let me know and I'll write you another.
It's amazing how dwarves can make a stack of bones completely waterproof and magmaproof.
It's amazing how they can make an entire floodgate out of the bones of 2 cats.
