Computer troubles

My main rig has been having trouble lately, various erratic behavior including display drivers stopping then beginning to work again, and screen artifacts followed by semi-shutdowns/black screens. It also appears to work for at least a decent amount of time after starting up before it happens again.
What I've done thus far:
replaced video drivers
cleaned out dust and monitored temperatures (all within stable temps)

I installed a hardware monitor, with one thing in particular standing out; the +12V on my PSU says it is at 10.03V; might this be the root of the problem? I've had the PSU for over 2 years now; unfortunately I don't have measurements from previous periods of time.

Edit: Incorrect monitor is incorrect. According to startup BIOS, PSU is fine >_>
Which leaves me back at square 1...

Sounds like a vid card hardware issue. The loss of power might (also) mean that the power connector to your video card might be bad in some way.
