My first death: dug too deep. As in, my miner was working too far away from my food stockpiles. He starved to death before he could walk all the way back.

Later, very recently after my first baroness, I had my first deaths by Goblin Ambush. I mobilize my militia and drive them off, but one of them dies from their wounds en route to the hospital. I bury him in a solid gold sarcophagus, as befit a hero who died in the line of duty (also because I had a ton of gold lying around). Later, my Baroness tantrums and I check out what it was that had been bugging her...

"Utterly traumatized at a filthy peasant's extravagant funeral."

Yes, she was horribly offended that a hero who died defending her home was given a burial with honors.

She... suffered an unfortunate accident shortly thereafter...

When my dwarfs vomited all over the front entrance when they spent some years inside. As well as the ocean waves impressed me.
