In the same way that miasma (a medieval medical theory) is represented in the game, why not all four of the bodily humors?
You got your blood, phlegm, yellow bile and black bile.

I thought that in-game miasma was just the  smell of rot nauseating people who smell it.

Yeah, miasma is just putrescants in the game, it doesn't represent the miasma theory. Applying a variety of alternative, optional approaches to healthcare would be pretty cool, though. If dwarven surgery was kept as the basic standard of healthcare, other approaches such as humorism, herbalism, acupuncture, diet-regulation and magical spells/faith healing (requiring specialist doctors with different skills) could be used alongside it to aid the chances of recovery. Even though most of the practices are debunked and laughable now, the placebo effect of being bled by leeches or having a magical spell cast over him should be enough to give the wounded dorf a little boost in his immune system, helping him to fight infections, at the very least.

Because our surgeons NEED MOAR LEECHES.
