Dying of thirst

I have plenty of food and booze and a well in my fortress, but my dwarves are constantly complaining of hunger, thirst, and I just had one dye of thirst.

Any ideas?

Complaining of thirst is normal (they won't go get booze unless they're bored or thirsty, and thirst -> complaining), but dying of thirst is weirder.  The one time this happened to me, I had accidentally killed a set of stairs between my miner and the booze.

If you're sure this isn't happening, make sure your booze isn't forbidden, etc.
