Dwarves will dig out down stairs directly below down stairs or up/down stairs, even though the stairs they dig out are inaccessible.

I wouldn't say that's a bug - YOU'RE the one who told them to dig that way, and they had access to the tile via the down-stairs immediately above it. I believe you can even channel the floor out 1 Z-level below a down-stair.

Ues Quietust is correct that is player error not a bug... use (I) for stairs

I dunno, it seems sort of buggy to me.  Or at least bad game logic.

I mean, how are they doing it?  Picture it in your head -- the down staircase definitely doesn't reach the floor of the tile below (they can't walk to there.)  So how do they manage to produce a staircase down into that floor, without building stairs in the intervening space?

This is a feature. Toady did this as a temporary measure until he puts in ladders.
