People who do that use drawing tablets, so it's almost like working on actual paper. Tablets allow the use of a stylus and are pressure-sensitive, so you can for example, bind the pen-width to how hard you press down on them. There's even laptops that have a screen and tablet integrated in one.

I've got one of those tablet laptops, they're quite fun to use. Especially in games such as Dwarf Fortress or Baldur's Gate... alas, the backlight died on the screen, so I have to attach an external monitor if I want to be able to see anything.

If you haven't done so already, and the laptop's not still under warranty, take it down to a repair shop and have them take it apart; you may get lucky and find that the lamp and/or power inverter are separate units instead of built into the screen, and therefore replaceable for somewhat less than what you paid for the machine in the first place.

Never used Dwarf Therapist, mods or tilesets in all the years I've been playing.
I think Toady's confusing interface better simulates the experience of a bunch of disorganised drunken dwarves running a fort.

I wish I could afford a new tablet, though I doubt I'd get better results than with my current one.  I just need to keep practicing.
