Adventure mode, human spearman. Fight with large rat and giant rat while trying to find a minotaur.
Spear gets stuck in giant rat and I lose it, so I grab a spare spear from backpack to finish the job. But when trying to take back the first spear, it is nowhere to be found, not at the body as usually, nor anywhere it could have flung to.
Could be related to getting the second spear stuck too, I suppose.

Okay, I'll note it down.  Seems like it'll be hard to reproduce though.

I just witnessed this in 39e.  After strangling a giant to death, my stuck-in battleaxe was nowhere to be found.  I suppose it's worth noting that I had lost possession of the axe, and wasn't holding on to it when the giant died.  It did give me an option to grab it in wrestling right up until the end, though.

(edit) Oh yeah, after the axe got stuck, I tossed an arrow at him and twisted it around for a while until he passed out.  So maybe it has to do with getting a second weapon stuck, like the OP's second spear?

I'd heard a rumor ( haven't been able to test it ) that if a stuck-in weapon disappears after the target dies, burning the corpse will return the weapon.  If you ask me, if it works, that's pretty cool.  Can anybody confirm?

I don't know if it's related, but I had a weapon disappear in fortress mode that may have been after it got stuck in an opponent.

I don't have enough information to know for certain, so it's probably not very helpful, but when checking the status of various combatants I noticed that an axe was stuck in a goblin, and some time later one of my axes had disappeared. I'm not even sure if it was the same axe, but I cannot think of any other reason for it to vanish.

maybe sticking a new weapon in over-writes the old weapon?
just keep sticking a monster with weapons....

Actually, it might be this one.  If the stuckin got skipped like a sock during the death drop, it would make sense that it reappeared when the body is burned.  So maybe the problem is already fixed for next time.
