I was just playing adventure mode and I got a quest from a Mayor to kill a named Alligator in a cave ("Dangershaft the Brilliant Vessel"). I go there and I get hit with pages of a generic ogre slowly torturing the alligator to death. After having the Ogre fufille my duty for me I go back to the mayor and immediatly get a quest to the same Ogre, who is now named.

The ogre may have already been named, names don't show up in combat reports.
