The hydra must have stubbed it's toe on that poorly marked stairwell.  Your fort is not OSHA compliant!

Foolprooof way to penetrate aquifers of unlimited depth.  (Make sure to import at least 10 stones for mechanisms)
Toughen Dwarves by dropping stuff on them.  (Nothing too heavy though, and make sure to wear armor.)

I think the stairs were a construction and it was trying to destroy it.  Or, the first bolt that hit it caused it to fall over and it never truly recovered.  I'm not sure.
