What about having Jeremy spray paint our name or an image associated with us around the city?  Maybe that can open some more mind ways.  Heck, maybe plan out some kind of mind rending symbol he can spray paint around?

Also we should make the world look nicer for him, more vibrant and alive.  Maybe inspire his creativity.

I think it is hard for a mental patient to leave the sanitarium.
Maybe we can make him appear more sane to others? But C for now.

As you bask in the devotion of the human, your body begins to grow yet again. You chiten cracks and softens, becoming a sort of rubbery material. It's still fit in plates over you, and has extended downward onto your legs. Your needle like teeth fall out one by one, giving way to sharp canines. They feel better in your mouth. You eye splits down the center, duplicating itself. You now have a regular looking set of human like eyes, except for the fact that each one still has two pupils. The bottom iris is a royal purple now, with the top one yellow-green. You horns grow in proportion but undertake relatively little change. The veins starting in you hands have become especially prominent and discolored, fading back to normal by the time they reach your elbows. You stand just short of 2 feet at this point, a minor imp indeed.
You notice a rather peculiar feature with your eyes. You can focus a sort of mental projection through them into the more immediate area, to create a hypnosis in your target. This only works from close range however, on one target at a time. (Hypnosis gained!)

Taking note of your physiological changes, you turn your attention back to Jeremy, your primary concern.

((Quick note here. He is a mental patient in that he sees a therapist for mental problems. He is not in a sanitarium))

You inform the poor man that his actions please you, that he should spread your name on the streets and hallways as well. He seems delighted to have curried your favor,  and takes to his task with apparent joy. +3 devotion. You must admit, it is very gratifying to have your name so readily displayed in this world, even if no one knows what it means. You brighten Jeremy's perceptions for it, easing his view of the world, soothing his fraying mind. When he describes his world to a vagrant, the man asks him where he buys his stuff from. odd. +0 suffering. You consider variations on his drawings, different things you could have him paint to spread your word. You are not currently associated with any images. You're a good deal too young to have any real affiliations anyways. Still, you suppose it's never to early to start building a reputation, even if it's just putting your name next to a picture. A better alternative you decide is to have him expand your network. The various scripts of hell hold great power to someone who knows how to read them. It could be possible to devise a symbol to act as an anchor for this particular mindway. Not even difficult really.
You would just need the proper name of your starting and ending locations, along with the coordinates. Something to bind them together, maybe a little flare to strengthen it. Should you name yourself in it? Your not sure what the exact effects would be, but it's not like your name is a secret right now. Maybe you would substitute it with a description, but ~>./..//~

Your thoughts are thrown into a flurry as something takes hold of you, wrenching your psyche out of the mind-ways like a root being torn from the earth. Your eyes roll back and your left side begins to spasm. Your psyche howls in pain and confusion. As you come to in the hellscape you become aware of a massive hand, larger than your head, clamped firmly over your face. You flail your limbs uselessly against your attacker, and bite down at the hand covering your face. Burning hot ichor fills your mouth with the taste of sulfur, but the hand does not budge. It seems an hour or more you sit there screaming silently, gnashing at the hand with your teeth, unable to see or move. At last, the hand sets you upright and removes itself to reveal another deamon. He is tall, nearly 9 feet, with great curved horns and a bulls face. His hands are massive and leathery and he cradles the hand you were biting.

You are not one to accept an impossible situation, little imp. Perhaps you might survive here after all.
With the size and strength of him, you decide it best not to attack outright again.
You anger is misplaced though. My actions have saved you newborn life.
He points at you with a massive finger from his undamaged hand.
You owe me now, little imp.

I am not so inclined to blindly accept these statements, demon. You reply as calmly as you can while spitting ichor from your mouth.
What favor have you done me with this? I see no present danger.

Feral demons. Few can follow the mindways anymore as you have. They turn to the anguish of other demons to sate themselves. Consuming flesh and ichor of thier own kind to grow in the absence of human worship. They have resigned to the fall of this world, unwilling or unable to venture outwards, they consume our very haven to delay thier own end but a little longer. I hid you, and myself, from them for they are fat and fearsome creatures, bloated and strong on the flesh of thier kin.

What of you, bull ape? You walk the mindways as well?

The demon grinns down at you. Not quite
He laughs as you scramble away, scooping you up and restraining you as you begin struggling again. 
Fear not little imp. I swore off the flesh of our kind long ago, but it is true, my mind is damaged. My path is hopeless in the mind ways. I shall grow no larger than my current form, but I shall die one day with pride as a deamon.

And what? You snap back.
You persist without anguish? It is impossible. You would wither and die. You hold knowledge of a path through the mind ways. You must! You Lie, Bull Ape!

 I never said I persist without anguish... he says, grinning down at you once more. You are suddenly aware of how tight his grip on your arm is, and a your eyes widen with a sudden realization.
Consider it payment, for saving your life.

Two hours later you lie in ditch of boiling mud, cradling the crushed remains of your right arm, you voice hoarse and scratched from screaming. The bull demon thanks you for your contribution before he leaves, and speaks one last sentence. I live in the caves carved into cliffs near your nursery. Come see me, if you seek information. You know what currency I deal in. And for your own sake, find a more private place for your meditations. It would be a shame if you were eaten so soon.

You are currently on a rocky outcrop at the edge of your nursery. You nursery is a vast pit writhing with larva and worms, submerged in a thin layer of a boiling viscous substance. The whole chamber is extremely humid. To the west looms a large wall of dark rock. Several caves cut themselves into the side of it, and the bull demon has disappeared into them. Behind the nursery, a massive staircase rises into the ceiling. It is impossible to see where it leads. A large set of sleek metal doors are set in the rock to the north. Some of the nursery is covered in large chunks of debris where some part of the ceiling has fallen in. All around on the floor and walls a thick blue green moss is growing. It glows faintly in the darker corners.

You have a little suffering from your escapades, but it will soon run out. If you don't want to wither you must find another source, either here in the decay of your own world, or back in the world of the humans. To meditate in the open brings with it certain risks however. There is also the condition of your arm to deal with..

Find a secure place to hide ourself. A cave with a small entrance that we can barricade ourselves inside would be a good choice.

Next to deal with our suffering upkeep, corrupt Jeremy's view of a man in such a way that he'll kill him. Do this in an alley or quieter place.
Have him do this in the most painful way possible but again corrupt jeremy's view so it doesn't affect him.

So, can we corrupt Jeremy so his madness or whatever starts returning, take away out protection, then blame it on some guy we can get him to kill?  Bonus to make the guy we tell him to kill look a little eldritch, not demonic, so he sees demons(us) as being the "good guys" and feels no guilt over the death.

(I'm not even sure we still have contact with Jeremy. Still, since we know that bigger demons like to torture/eat smaller demons, I think we should try to raise some worms into tasty imps to sell to bigger demons.)

Head to the Nursery, and spend some time interviewing the Worms. Find and befriend few who are not particularly bright, and give them names. Ideally we can get humans to worship them, so that they might grow big enough to serve us or give as offerings to the Bull Ape to torment.

Once we have the names of some Worms, seek out the Bull Ape's Lair. Offer to let him break our other arm in exchange for the right to meditate in his home under his protection. Then explain that we intend to use humans to help a few dumb worms grow, so that they might be subjected to torment in our place as future payment for his services. Since our scheme would benefit him, he might be willing to share some free advice on the matter.

Oh this seems fun but I have one question, how were we able to grow to a minor imp without  really causing any suffering at all as far as I could tell ???

Go back to Jeremy see if we can get him to do something that may cause suffering without getting the human authorities unduely interested in hi like say carefully breaking windows around town, this might not cause physical anguish but people will at the very least be annoyed/slighty scared by the random property damage. Additonal perhaps trick him via presenting an illusion into also urinating into the rooms through the holes in these broken windows, convince him that he has done nothing too unusual and that his actions, whatever he can remember of them please you greatly.

If that's not enough, Idk get him to throw a rock into a chained dog or similar.

Investigate the other worms for future suffering value as in Weirdsound's idea.
Then, our devotee shall be ordered to find appropriate vessels to feed our need for suffering; in particular, if he could try to form a romantic attachment to another human, it would be easy to cause that human a great deal of suffering without even doing anything overtly criminal.
