Double EDIT: I think I fixed it.
Apparently you need [DOES_NOT_DETERMINE_PRODUCT_AMOUNT] after reagents that accept stacks, or else the game tries to use multiple. Because it was using up a partial stack, it was causing it to keep reacting until the bar was used-up.

The missing tag caused multiple output items. It was actually only pulling one bar for use, though, because it turns out bars are calculated differently.
1 "bar" is NOT 1 piece of metal. A bar is actually an alias for having a stack of metal. A full stack is 150/150 units.

Therefore, a metal reaction that takes one bar needs the reagent token to be 150, not 1.
