So how do you get them to build up to a second floor?

Probably by including one or more (permanent) stairways inside the wall. On each new level you can mass-build the central floors by using the floors provided by the walls below, then use the central floor to mass-build the walls on both sides. Construction should proceed as quickly as dwarf-power / supplies allow.

You can accomplish something similar (but more resource-efficient) by staggering the wall, allowing your dwarves to use the top of each level to construct the next. Staggered walls are still considered impassible in most cases, though there is a bug that can allow creatures through if twigs or stairways are on top of a constructed wall.
