I think that a dwarf is only counted as buried if every bit of him is in the coffin.

So if there are a pile of dwarf teeth in a corner somewhere you may get ghosts.

I've had as little as a mangled partial skeleton put it to rest and other times couldn't even tell what was missing but it didn't.  Memorial slabs always seem to do the trick, though.

Issues with Urist McCatperson wanting to bury fluffy right away, instead of putting urist mcdumbass in it to make his ghost go away.
Toggle off pet burial. Why would that be on.

I am aware of this.  The problem is pet burial tends to be enabled by default.

The happiest dwarf in the fort just happened to be a vampire, who was locked in a 2x2 earthen room with a masterwork table, chair and door. Nothing else to look at for years=Very happy!
Then a ghostly pet began haunting him. It took years to break him!!! Never saw that ghost more than 10 tiles away, and always returned to "vamp2". Vamp2 eventually took out the door and met vamp1 who promtly beat him to a pulp, but didn't kill him!  Laying paralized on the dirt, surrounded by the full coffins, and that ghost STILL haunted him!!!
He's no longer a happy vampire.  :P
