I am experiencing this same thing.  Therapist shows one of my dwarves killed 53 others.  Not a vampire, has no listed kills ingame, tons of combat logs of him slaughtering everyone, but no reasons, no event of him going berserk, no crime.

But are there combat reports? Mine didn't show any combat reports and I started a new fortress in .03 and I haven't had any deaths so far.

Your case sounds like it could be a dwarf enraged by booze.
