So I've been out of the loop

I've been thinking of getting back in to DF after being gone for a few years, what kind of big changes are there now? I think the last major change that I saw was the introduction of carts, and the reworking of weight and move speeds.

Minecarts and such would be 0.34.08. Since then, we got -
* World activation - creatures now get born, married, armies attack sites, and people die off-site and assume entity positions as needed.
* Adventure mode visitable forest retreats, dark towers, and mountain halls (and spoiler sites as well).
* Multitile trees, and a few tons of plants.
* Fortress retirement (and unretirement)
* Adventure mode climbing, jumping, and tracking, and updated movement/stealth mechanics.
* Combat has threat levels now, differentiating between fisticuffs and lethal combat among others
* Adventure mode conversation and recruitment overhaul - it's more of a action/reaction now, and NPCs talk among themselves (quite a lot)
* The old thoughts system was replaced with an circumstance/emotion system that tracks stresses.
* Civilizations have values now that lead to individual goals
* Certain creatures can now be gelded (neutered), and reproduction requires contact.
* Job priorities got reworked and should get more important jobs quicker now. There's also a Do it now option for building tasks (workshops/levers)
* Designations can be given priorities from 1 to 7 (1 most important, 7 least), and can be places as markers (and be toggled).
* Mining designations can be switched to valuable stones only and be set to follow veins automatically.
* Wall/floor/road building uses a skill-less labor rather than masonry/carpentry etc.
* Haulers can generally carry multiple small items.

So with world activation, can I now dam a river and then downstream of the river turn into a dry wasteland? or at least have the river dry out down stream of the dam

No, world activation if pretty much limited to the things I listed at the moment, and while  the list will only grow, I'm rather doubtful that damming or diverting rivers will work even in the future.

You can think of world activation as the making of history while you play - events, battles, deaths, all simulated and fairly irrelevant to your immediate task of building a fortress, but very important for conveying the feeling of reality in these fantasy worlds.
