Dwarves always drowning?

My dwarves keep drowning in this (rather large) river that I use as a moat around the city walls. Why do they keep ending up there?

There is nothing to frighten them, they are not training there, there aren't even workshops or anything on my city walls.

They climb down the walls to drink and/or wash probably. Or, if there is no alternative route and they have something to do outside, like plant gathering or wood hauling, they WILL try to climb and swim.

train the idlers in swimming via stupid dwarf trick. Like that minecart underwater track, or simply dropping dwarves into a 5/7 water pool. 6/7 is fine too but if you have 7/7 tiles in your 6/7 it's going to become fun.

Or just set up two dwarven childcare facilities: one for the children, one for untrained adults. Voila, swimming and dodging skill become legendary within a year or three.

Dwarves which see a creature they don't like can get vengeful thoughts, and chase them from the other side of a river.

Has happened to me, with dorfs wilfully drowning themselves attempting to reach undead giant animals on the other side. Which they intend to get into a fist fight with. Silly dorfs.
