As the title says, how do I select the nearest creature in a reagent.

You cannot. You can only specify items in the reactions using only reaction and item tokens. The wiki is completely and utterly comprehensive on this.

Then how do fell moods work. Maybe I misphrased my question, I want to transform a dwarf a workshop into another creature and I assumed you would use a reaction.

I'm fairly sure that they don't even subscribe to the idea of a "reaction". It's just another hardcoded job type, on line with "make bed" and "shear animal".

Fell moods are not a reaction. Reactions are strictly taking items as reagents and producing items as products.

If I can't transform creatures using reactions, how do I transform them on command in fort mode?

could I give a material with a contact syndrome a very low boiling point and produce it only through a reaction, and then the dwarf who created the material gets caught in a burst of the contact syndrome material and transforms.

would the temperature update when the material is created or would you have to wait for something to happen to it?

The temperature updates just fine.  The issue is that it's quite hit-or-miss whether the guy doing the reaction is affected by a contact or inhalation syndrome, which makes it finnicky for the player to use and hard to balance as a designer (if it's supposed to be a rare thing).  This is why DFHack has tools like autosyndrome nowadays.
