Dwarf not being buried

So, a while ago one of my dwarfs got a strange mood. This was early on in my fortress' life, and he couldn't find the workshop he wanted so he went insane and starved in my glorious dining hall. I honestly didn't care at all (He probably makes a decent chair) until his miasma started stinking up the place. I have built several coffins (at least a dozen) and set them all to be used in burial. the first coffin I placed in my catacombs even says that that dwarf owns the coffin (I never assigned it to him however). Despite all of this his partial skeleton is still rotting in my dining hall, and now his ghost is haunting me.

Do I need to make a memorial slab for him? or am I doing something wrong with the burial process?

Note: it is not a labor issue, my dwarves have burial, refuse hauling etc turned on.

Nevermind; it turns out when he first died I set him to be dumped and nobody bothered to do so. as soon as I reclaimed him he was put to rest.
