I have a habit of placing a memorial slab to every notable kill my militia captains achieve. Unfortunately, while my captain was hacking the limbs off a kobold, my commander rushed up and stabbed the kobold in the face. The game gives the kill to the commander, but I'm not sure who to give the memorial to, as the captain would have been fine without the commander's help.

I'd go with whoever the game credits.  "Kill" implies causing death.  Sure your captain was whittling the kobold body into kindling, but the other dwarf ended his pathetic existence.

Of course, the decision is yours.  If you really wanted you could record "assists" or split deaths--kinda like getting 1/2 a sack in football.

Btw, I like the general idea of a slab memorial of kick-ass-dom.
