Questions about the map

Is it just me or does the map make no sense at all, when ever I check the map to see where to go for a quest objective it seems like an entirely different area does it actually show where I am? or just the objectives, when I try to move in the direction the map is showing me I end up in a completely different area all together.

Any help would be appreciated thanks

The map zooms in and out sometimes. When you get near a city it zooms in to show where the houses are, and when you leave it zooms back out. The [Q]uest map is always zoomed out.
